My site is all jumbled up and I need help with it soon

 I can't log out and stay constantly logged in.  I can't access the Admin CP to look at this either.  I can get into the MOD CP.  The site shows undefined index uid, PHP file, etc and it's all at the top of the screen and you have to scroll down long way to find the area to post and read.  Please help, Jon.   Thanks

Cannot even get a new password either.   I used activation code and it's not working either.
